Hopefully you all find this form easy to complete but any questions please reach out to your age group contact or to Eamon Clarke (0863871996).

Should you wish to order a gear pack, then one order form should be completed for each player- parents with more than one player in the family should submit a form for each girl. Once all orders forms are received we will let parents know what the final cost of their order is (indicative is €50-60 up to ages 13 and €65-75 for older groups)
